No, holders of distinguished residency are not exempt from medical insurance, but rather it is a basic condition for obtaining and maintaining residency.

Article Six of the Distinguished Residency System explicitly stipulates that the applicant for distinguished residency must do the following within (30) days of being notified of approval:

B- Submitting a medical insurance policy in accordance with the Cooperative Health Insurance System and its executive regulations

This was confirmed by Article 6 of the Executive Regulations of the Premium Residency System, which requires that the medical insurance policy for the holder of premium residency and his family members be valid for the duration of his stay.

This commitment is consistent with the primary objective of the Cooperative Health Insurance System, which, according to Article 1, aims to provide and organize healthcare for all non-Saudi residents in the Kingdom. Article Two of its executive regulations explicitly subjects “non-Saudi residents of the Kingdom” to compulsory health insurance, and holders of distinguished residency fall under this general description.

Based on the above, obtaining and maintaining valid medical insurance is a legal obligation in which there is no exception for the holder of privileged residence and his family.
